You will need to get on the Performance List and fill in the NHS Transfer form. This takes 8 weeks and you will not be paid and cannot see patients until this process is completed.

In addition you will also need to have a CRB check, for this you will need to contact Rose Achieng at the PCT Tel No 0207 525 0487( to arrange a suitable time to see her. Attached is a booklet giving you details of what you will need to provide.

When you join a practice the practice manager may need to see your Medical Defence Certificate, Hep B check. You will also need a Smartcard and NHS email address. The smartcard will again require you to go to the PCT to have your photo taken and again you will need id – see above.

You can still use the NHS smartcard from your previous trust. But you would still need to fill out the following forms an RA01 and an RA02 form signed by the appropriate sponsor. You would also need to bring in 1 photographic ID e.g Passport or driving licence, 2 proof of address (within 6 months of today date) e.g bank statement or utility bill. You will need to make an appointment with the practice to come in and have this actioned for you. You can find the appropriate forms on the intranet:
